UPMC Home Health is hiring a Senior Professional Staff Nurse to join its team and support Zelienople, Harmony, and the surrounding area's.
This is a Full-Time position working daylight hours with rotating weekends and holidays as well as on-call shifts.

Responsibilities:
- Initiates and coordinates case management activities within a multi disciplinary health care team to insure appropriate delivery of services to patient.
- Interdisciplinary relationships are fostered by negotiation and consensus building for attainment of goals for all.
- Initiates, coordinates and insures ongoing communication with the physician and other members of the health care team.
- Role models assertive and professional communication strategies and holds peers accountable for similar behavior.
- Attends and participates in patient care conferences, team meetings and other related meetings.
- Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the home care agency towards patients, visitors, staff, peers, physicians and other departments within the organization.
- Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.
- Serve as a preceptor for new staff members and role model preceptor responsibilities.
- Function as an expert within clinical specialty areas of certification and incorporate evidence-based nursing into projects and practice environment.
- Attend compliance training and adhere to organization standards of conduct, policies and procedures.
- Focus on customer service and continually strive to perform job duties in a manner that results in optimal patient satisfaction.
- Assign other nursing personnel, including LPNs and Aides to assist in the appropriate delivery of services as ordered on the plan of care to the patient.
- Supervises and teaches other nursing personnel including LPNs and Home Health Aides.
- Influences behavior to create positive change and utilizes alternative leadership styles as necessary.
- Identify, investigate and recommend solutions to organization issues.
- Identify any deficits or exceptional performance issues of other nursing personnel, LPNs and aides, and recommend solutions.
- Provides creative and innovative approaches to direct patient care, evaluates outcomes and adjusts nursing care process as indicated to ensure optimal patient care.
- Assumes responsibility for complex patient assignments; views problems as challenges and helps peers to do likewise.
- Initiates, coordinates and evaluates patient and family centered health teaching.
- Establishes the plan of care in collaboration with the physician, patient and UM Professional in accordance with the physician orders and patient needs.
- Advises and consults with family and other agency personnel, as appropriate.
- Completes documentation including visit assessment, OASIS, 485 and Verbal Orders accurately and promptly on the laptop device or in written documentation while in the patient's home and within the agency's and regulatory standards.
- Data syncs (transmits) information same day.
- Anticipates/responds and exercises independent judgment in matters concerning emergent and non-emergent patient care needs and communicate with the physician.
- Participates in on-call coverage rotation for the agency.
Qualifications
- Graduate of an accredit school of professional nursing required.
- Home Care Agency approved certification preferred.
- A minimum of two (2) years of experience in Acute Care Facility or Skilled Care Facility or three (3) years of Home Health nursing experience
- BSN Preferred
- A current Home Care Agency approved Nursing certification.
- UPMC's tuition assistance program supports certification.
- Nurses with a master's or doctorate degree in Nursing are not required to achieve certification.
- On an annual basis, nurses in this position must identify and successfully complete a pre-approved professional contribution that supports UPMC's nursing goals.
- Examples of professional contributions include a patient satisfaction improvement initiative, a work redesign project, recruitment or retention initiative, or a staff development initiative.
- Successful completion of two (2) standard professional contributions or one (1) standard contribution and UPMC Home Healthcare approved national nursing certification required (Master's degree in Nursing (MSN) may be used in lieu of certification)
Licensure, Certifications, and Clearances:
- Current Pennsylvania licensure as a Registered Professional Nurse.
- UPMC approved nursing certification required. Must maintain certification status.
- Certification not required if Master's in nursing is held.
- CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
