Purpose:
UPMC Home Health is hiring an RN for the South Hills area working Monday - Friday daylight shifts. You will work one-on-one with your patients. This position provides flexibility and autonomy. Ability to work with patients one to one and see them progress in their own environment.
Ability to participate in the Nursing Career Ladder. Opportunities to participate in in-depth Wound Care and Palliative Care education. Opportunity to really know patients and help them move to healing and wellness. On call and weekend rotations may have travel to Washington, Clairton, and Mon Valley.
Responsibilities:
- Attend compliance training and adhere to the organization standards of conduct, policies, and procedures.
- Visit patients in their homes to provide physician ordered care
- Document care provided, patient response on Clinical record via computer system
- Maintain compliance with all Home Care policies and procedures
- Case manages a group patient with a variety of health concerns and nursing requirements in a geographic area.
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Assigns other nursing personnel, including LPNs and Aides to assist in the appropriate delivery of services as ordered on the plan of care to the patient.
Qualifications
- Graduate of an accredit school of professional nursing required. Bachelor's degree preferred.
- UPMC Home Health Care approved certification preferred.
- Six (6) months of nursing experience in an Acute Care Facility or Skilled Care Facility
Licensure, Certifications, and Clearances:
- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
- Registered Nurse (RN)
- Act 33
- Act 34
- OAPSA
