Summary:
UPMC is hiring an LPN for the Family Hospice and Palliative Care team. You will support the Pittsburgh and East communities working Monday - Friday 8:00 a.m.- 4:00 p.m. We are excited to announce the expansion of the LPN role to strengthen the holistic care that we strive to provide to our patients and families each day. Responsibilities:
- The Hospice LPN will provide care to patients through an interdisciplinary team approach through collaboration with the RN Case Manager, Home Health Aide, Social Worker, chaplain, volunteer services and physician.
- Visit patients in the community setting or nursing facilities to provide assessment of the hospice patient's physical, emotional, and psychosocial concerns
- Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record.
- Promote quality of care by providing patient education materials, communicating physician advice/instructions, family support
Qualifications
- Successful graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing.
- Experience in health care preferred, no LPN experience required.
- Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Mobility and visual manual dexterity. Physical stamina for frequent walking, standing, lifting, and positioning of patients. Computer skills required to utilize the electronic health record.

- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
