The PsychCare Plus team at Western Psychiatric Hospital is hiring a Behavioral Health Navigator Supervisor!

The Behavioral Health Navigator Supervisor is responsible for the assessment and navigation of patients and families in collaboration with attending psychiatrists and other staff within a psychiatric service either in person or virtually/telephone. The incumbent will develop initial treatment plan to facilitate disposition planning and assists with bed placement or disposition planning.  They will also facilitate the flow of information to promote effective communication regarding the patient care and patient operations and disposition, and functions in ED, ED and medical inpatient area or PsychCare+. They will have the ability to identify acuity level of patient and will provide supervisory support to the department.

The incumbent will primarily work Monday – Friday, with on-call responsibilities on the weekends.  This position is based at Western Psychiatric Hospital, but travel will be required to other hospital locations that participate in the Behavioral Health Navigator program. This position is a wonderful opportunity to gain leadership experience and collaborate on enhancing behavioral health services. 

Responsibilities:

  • Complete patient/family assessments referred to emergency services for presentation to the attending/resident psychiatrist or within Psych Care Plus, review patient material for possible IP admission or conduct evaluation for outpatient psychiatric service.
  • Meet standards for documentation and completes all required paperwork, including psychiatric assessment, commitment paperwork.
  • Demonstrate ability to utilize DSMV and to develop provisional diagnosis.
  • Demonstrate knowledge of mental health laws and reviews inpatient commitment for accuracy, competencies, and proper time frames.
  • Obtain appropriate consents or commitment authorizations as needed.
  • Ability to work efficiently and independently in emergency setting and medical setting.
  • Knowledge of insurance programs, processes and able to obtain authorizations as needed.
  • Manage crisis situations effectively using positive approaches.
  • Assist in maintaining safe environment.
  • Complete assigned projects, reports, and statistics in a timely manner.
  • Participate in interdisciplinary activities such as staff meetings and other committees as assigned.
  • Maintain competency requirements.
  • Serve as mentor to students and new employees.
  • Learn and utilize up-to-date technology in delivery of patient care for assessment and disposition/referral planning.
  • Provides BH and patient management education and treatment planning.
  • Efficiently take a sizable volume of calls and disposition callers to meet their needs rapidly
  • Follow outpatient scheduling or referral documentation guidelines
  • Responds rapidly to appointment or inpatient admission requests through email, phone, or voicemail
  • Review and summarizes the patient information for admissions to a behavioral health unit and triaging patient for appropriate placement
  • Ability to verbally present the case to physicians and others
  • Demonstrate triage and assessment skills for medical necessity or appropriate outpatient clinic
  • Provides accurate, timely information to coworkers, physicians, outside agencies, with a professional and courteous manner
  • Active participation and ongoing maintenance of quality management activities and the development of standards from problem identification
  • Demonstrate the ability to identify, assess, counsel and effectively intervene with medically and psychosocially complex patients with the ability to understand complex dynamics related to the clinical situation.
  • Successfully utilize therapeutic counseling methods such as crisis intervention, psychosocial support, brief goal-directed therapy, CISM and/or other appropriate therapeutic methods that may be appropriate to the clinical situation.
  • Collaborate effectively with the healthcare team, patients and families and relevant community resources to develop and implement and individualized plan of care.
  • Demonstrates expertise in accessing social systems by providing resource-specific information to patients and families, initiating contact with appropriate resources and facilitating the patient/family’s ability to accept referrals
  • Provides Lead responsibilities for the shift by providing support, direction and problem solving. Provides change of shift hand off and assures all documentation is complete.
  • Provides one-time on -the-spot coaching interventions for other team members.
  • Provides timely feedback to manager concerning performance of peers 1,2 3.Identifies and reports any clinical, utilization, provider or outcomes issues to supervisor and offers recommendations for improvement.
  • Utilizes independent problem-solving and demonstrates ability to propose and implement creative solutions to problems to achieve a high level of satisfaction with services.
  • Provides mentorship to other navigators and assists with special projects as assigned.
  • Serves as a leader in clinical practice, identifying patient and staff focused opportunities for improvement and leading the change process.
  • Identifies, educates, and supports the psychological and learning needs of staff during change and transition.
  • Practices participatory leadership and is an active member of the leadership.
  • Assumes formal leadership responsibilities for one or more of the following: shift responsibilities. quality improvement, scheduling, new hire on-boarding and/or ongoing education.
  • Completes and/or assists with staff performance evaluations for direct reports as applicable.

Qualifications:

  • Bachelor’s Degree in Psychology, Counseling, or Social Work, with four years mental health experience required at time of hire.
  • Master’s Degree in Psychology, Counseling, Social Work with four years mental health experience required.
  • Other fields of study may be considered with coursework of at least 12 credits in human services and/or relevant mental health experience.
  • If the Master’s Degree is not present at time of hire, must be enrolled in a Psychology, Counseling or Social Work Master’s program within one year of hire/transfer date and completed within three years of enrollment.
  • Other fields of study may be considered with coursework of at least 12 credits in human services and/or relevant mental health experience.
  • Licensure in social work or professional counseling is preferred.,
  • Experience in a supervisory/charge position preferred.
  • Ability to respond calmly and effectively, using good clinical judgment to both psychiatric and medical emergencies.
  • Ability to identify problems, implement resolution, reassesses situation/problem.
  • Excellent interpersonal relations with the ability to interact in a direct, professional, tactful, gracious manner with all professional/non-professional contacts; includes all areas of verbal/non-verbal communication within the contest of direct/non-direct contact; history of excellent interpersonal relations.
  • Ability to work collaboratively within a multidisciplinary team.
  • Ability to work under stress, with an unpredictable patient flow, in a sometimes-crowded work environment.
  • A personal disposition that is warm, friendly, caring, kind and hospitable towards patients, colleagues.
  • Exhibit openness and willingness to learn.
  • Flexible schedule
  • History/record of all above services consistently delivered with a service excellent attitude.
  • Ability and willingness to work in a managed care environment.
  • Excellent ability and history of direct, gracious, courteous, kind face to-face, telephone and nonverbal skills.
  • May in the course of their job duties be in rooms where medications are stored.
  • No contact with medications is allowed.
