UPMC Hamot is currently seeking a dedicated and compassionate Full-Time Professional Staff Nurse to join our Transitional Support (4 North) unit team. This is an excellent opportunity for a nurse passionate about providing high-quality care in a dynamic, supportive environment.

About the Transitional Support Unit (4 North): Our Transitional Support Unit (4 North) is an 11-bed unit that provides care for medical and surgical patients who are medically stable but awaiting discharge. This unit is an ideal setting for nurses who have a passion for long-term care nursing but also want the experience of working in an inpatient hospital environment.

Position Overview: The Professional Staff Nurse is a Registered Nurse and a key member of the care delivery team. This role is responsible for setting the standards for the level and quality of care. The Professional Staff Nurse holds responsibility, authority, and accountability for the provision of nursing care. The nurse will manage and provide patient care activities for a group of patients and their families by applying independent judgment and collaborating with all team members. This role encompasses leadership, partnership, collaboration, and supervision, and the nurse will establish and maintain collaborative relationships with physicians, healthcare providers, patients, and families to achieve optimal patient outcomes throughout the continuum of care. The Professional Staff Nurse also demonstrates a strong commitment to both the community and the nursing profession.

Previous nursing/rehab experience is highly preferred. Job title and salary based on education and experience.

The minimum rate of pay for a non-BSN Professional Staff Nurse is $33.75.

Responsibilities:

• Patient Care & Relationship-Based Care: Applies the nursing process within the framework of Relationship-Based Care to create a healing environment. Develops individualized care plans that involve patients as partners and address their holistic needs. Exhibits critical thinking when identifying clinical, social, safety, psychological, and spiritual aspects of patient care. Ensures clear communication with care team members and patients regarding conditions and necessary changes in care. Ensures comprehensive patient documentation to support communication across care teams.

• Patient Education & Teaching: Demonstrates knowledge of adult learning principles (and/or teaching children if applicable) and applies them when teaching patients, families, students, and new staff. Provides detailed, patient- and family-centered education to guide individuals through their care journey and transition to another care level or home. Supports the development of students, new staff, and colleagues and may serve as a preceptor.

• Quality Improvement & Patient Satisfaction: Actively participates in quality improvement initiatives within the department or unit. Identifies opportunities for improvement and collaborates with colleagues and management to enhance patient satisfaction and care quality. Utilizes research and evidence-based practices to support clinical care improvements. Advocates for quality care and safety during multidisciplinary rounds and patient care discussions.

• Teamwork & Collaboration: Develops and maintains productive relationships both internally and externally by demonstrating accountability, motivation, and commitment to colleagues and patients. Demonstrates cultural competency and promotes healthy interpersonal relationships within the team. Strives for a balance between work and personal life, modeling safe work hours and time management. Promotes a healthy work environment by identifying and communicating safety hazards.

• Additional Responsibilities: Performs other duties as assigned in alignment with system-wide competencies/behaviors.
