Social Worker – MSW

UPMC Hamot is seeking a full-time Social Worker (MSW) to serve Monday through Friday, 8:00am – 5:00pm. We offer comprehensive total rewards and up to a $6,000 sign-on bonus. The ideal candidate will have experience in end-of-life or hospice care and possess strong emotional resilience due to the nature of the work. This close-knit service line collaborates seamlessly to advocate for patients and achieve the best possible outcomes. End-of-life care at UPMC Hamot directly contributes to our cultures of caring and listening, dignity and respect, and responsibility and integrity. It is crucial that the candidate upholds and models these behaviors daily. This role involves collaboration with various departments, consulting across different service lines, and interacting with multiple physician specialties and nursing units. The Social Worker will have the opportunity to enhance UPMC Hamot’s patient experience through HCAHPS scores and impact ongoing metrics such as length of stay.

In this role, there is the opportunity to – 

• Provide resource-specific information to patients and families, initiating contact with appropriate resources and facilitating acceptance of referrals.
• Maintain extensive knowledge of federal, state, and local assistance programs and community resources affecting patient needs.
• Mentor social work students and/or serve as a field instructor to MSW students.
• Utilize therapeutic counseling methods such as crisis intervention, psychosocial support, and brief goal-directed therapy appropriate to the clinical situation.
• Document all social work interventions accurately, timely, and appropriately in the medical record per regulatory policies and procedures.
• Educate patients and families regarding payer requirements and coverage for post-acute care services, advocating effectively to obtain
• authorizations for continued care.
• Identify, assess, counsel, and effectively intervene with medically and psychosocially complex patients, understanding complex dynamics related to the clinical situation.
• Monitor and evaluate the plan of care, options, and available services throughout the hospital stay, reassessing as status changes or at least every three days.
• Demonstrate professional practice, maintaining respect for confidentiality and freedom of choice as outlined by the Code of Ethics by the National
• Association of Social Workers and the State Board of Social Workers, Marriage and Family Therapists, and Professional Counselors.
• Collaborate effectively with the healthcare team, patients, families, and relevant community resources to develop and implement an individualized plan of care.
