Full-Time Clinical Intestinal Transplant Coordinator (UPMC Presbyterian Montefiore)

We are seeking a dedicated, experienced Registered Nurse to join our team as a Clinical Transplant Coordinator in the Intestinal Transplant Department. This meaningful role supports patients throughout their transplant journey and plays a vital part in life-changing care.

This position works Monday through Friday, 8am-4pm, and offers a hybrid schedule with a mix of in-office and remote work.

Purpose:

The Clinical Transplant Coordinator I is responsible and accountable for delivering nursing care using the nursing process. In collaboration with fellow coordinators, leadership, the transplant team, and other healthcare professionals, this role applies sound clinical knowledge and skills to achieve high-quality patient outcomes.

The Clinical Transplant Coordinator I demonstrates entry-level competencies to provide age-appropriate care, incorporating principles of growth and development across the lifespan. This role also requires the ability to assess and interpret patient data to identify and address each patient’s individual needs.

Responsibilities:

Referral & Evaluation

Coordinates the evaluation process for transplant candidates, working closely with the multidisciplinary team. Identifies candidate needs, advocates for patients and families, and ensures compliance with regulatory and UNOS listing requirements. Submits required data to regulatory agencies.

Pre-Transplant Care
Develops and delivers patient and family education, maintains communication with referring providers, and monitors candidates for changes in condition while awaiting transplant. Serves as a clinical resource and ensures accurate documentation and maintenance of transplant listing status.

Peri-Operative Coordination
Supports the transplant process according to UPMC protocols. Reviews recipient medical status, facilitates admissions or transfers, collaborates with organ procurement organizations, and ensures appropriate waitlist status updates per UNOS guidelines.

Post-Transplant Care – Inpatient & Outpatient
Monitors patient progress after transplant, provides education to recipients and families, and collaborates with the transplant team to implement care plans. In the outpatient setting, assesses ongoing needs, reinforces follow-up care, monitors organ function, manages documentation, coordinates consultations or hospitalizations, adjusts medications as ordered, and promotes treatment adherence.

Pursues ABTC certification, participates in conferences and outreach activities, engages in ongoing learning, responds constructively to feedback, completes self-review activities, and fulfills on-call responsibilities.
